Elise found herself aboard a dark train, confused but reunited with the rest of the group, who showed signs of injury from the preceding battle but were still alive. As the train rolled on, they learned that they were in the mountainous West Virginia stretch of a line that ran from Baltimore to Cincinnati. Elijah negotiated for cheap tickets from the conductor. A passenger looked out the midnight window and remarked how strange it was to see snow in late April. While Sagesse and Elijah attempted to flirt with passengers, Caleb cheated at a game of poker, and Elise discovered signs that the train was brand new. A scream brought them to a sleeper car, where they saw a man be stabbed in the back by a floating knife. Before Sagesse could tend to his wounds, the man behaved strangely, smashing his telegraph machine before dropping dead. Elijah spotted a reflection in the window of someone not in the room. The screaming woman indicated that an invisible intruder had attacked her companion, the telegraph officer of the train. The inspector initially accused the group of murder, not believing their claims of ghosts and floating knives, until Caleb manipulated him to accept their story. The group saved the inspector from a floating gun, but Elise's exorcism appeared to anger the spirit, who threw objects about the various train cars, then sped up the train and ran it off the rails. Elise and Elijah dodged flying glass and cutlery in the dining car, while Sagesse and Caleb toppled into the corner of a sleeper car. Clattering thunderously, the train careened down a rugged hill and slipped over the edge of a cliff.

The group survived the train wreck, but only just barely: A battered Elijah clung to life, continuing to see a strange man in his reflection. Sagesse treated survivors and learned from the conductor that the same crash had happened on January 2; this new train was the first to repeat the route after the company declared it safe it again. Caleb rescued trapped survivors, discovered that the engineer's neck had been snapped, and aided a search party from the nearby village of Falls Run. The group found the residents of Falls Run to be welcoming of the stranded outsiders, but were warned by a teen that it was a repugnant and deceitful place. Sagesse was alarmed by the lack of support for the local doctor, who was ostracized for being an atheist, and conflicting stories about what happened in the previous crash three months earlier. Elise heard stories about a haunted house and everyone in the tavern suddenly falling asleep. Caleb learned that the miners were not allowed deep into the cave, for some sections were off-limits. When something compelled Elijah to say, "I'm thirty, let's go to the tavern," the group broke in and found an occult meeting room in the basement, containing the stuffed and mounted body of a reporter from the January wreck who asked too many questions around town, the same man Elijah kept seeing in reflections. Elise found a hidden door that led them deep into the mine beneath the town, where they found an icy chamber blocked off from the rest of the mine. Under solid ice, they discovered seven frozen bodies: Alexandria Knight, Celio Parisi, John Sheppard, Aidan Kane, Katerina Petrovich, Lawrence Tate, and Elise herself.

The group dug Elise out of the ice and wiped away the occult symbol on her neck, waking her. She reported being drugged at a party in Baltimore on New Years Eve, with no memory of the interim. One by one, the rest were awoken and told the same story: Alexandria demanded to leave, Celio asked if he would be paid for the party, and Aidan objected to being tied up. When the group asked John about a Latin spellbook that they had found upstairs, he translated it and began casting spells on the group, and had to be compelled to undo the damage. Another book describing summoning beings from a parallel world, which led to speculation that these people were brought from another world to this place. When the man in Elijah's reflection warned him that danger was coming, they went upstairs and were confronted by seven armed townspeople, who called themselves the Sleeping Wolf and said that someone named Jessik was their leader. Elijah pointed at a man brandishing an ax and declared, "That's the man who killed me!" The Sleeping Wolf announced their intention to kill off the group for invading their secret space. Elijah, already seriously injured, fled to save his life. Elise crushed two of the qabalists, but was compelled to flee with a spell. Sagesse used her spellbook to turn invisible. Caleb was magically compelled to surrender, and was knocked unconscious.

On a foggy mountaintop, Caleb had a dream-vision of a man in red robes staring at a clock, who warned them that they were running out of time to kill "the twelve." The man claimed to have sent them to San Francisco, where they had failed to kill Dracula for him, and now they had to kill someone in Falls Run before the Red Death's progress could become insurmountable. Caleb awakened in the midst of a fight in which Sagesse found herself enchanted and Elise wreaked havoc with magically thrown boulders. After defeating the Sleeping Wolf, the three of them found the rest of the party missing from the ice cave. Trekking deep in the mines, they found the leader of the qabal, Jessik, a wolfwere disguised as a beautiful, smiling woman. She admitted teaching the people of Falls Run magic spells to serve as her private army. However, she denied having anything to do with the seven "doppelgangers," who were in the original January train wreck, being shipped from Baltimore to a Dr. Caligari in Vancouver. The ghost of the murdered reporter demanded vengeance before his soul could rest, saying that he had caused a repeat of the original train wreck and stranded the group in town to force them to confront his killers. When the group refused to join Jessik's qabal, she transformed into a wolflike form and attacked, slaying Elijah who disappeared in an instant. The combined forces of the surviving party defeated Jessik in a bloody combat. (more detail)
